Case Manager/ Therapist/ QMHP /QIDP

Job or Role Description: 
The Case Manager/ Therapist is responsible for ensuring the development, implementation, and provision of needed services for clients enrolled in Centerpoint programs. This includes ensuring the timely and accurate completion of required documentation for each client served. This is a full-time position.

Qualifications:

• Specialty Certified- Social Worker
• Minimum of a bachelor's degree in social work, or Human Service field.  
• Willingness to learn and attend ASL classes. 
• Possess valid Driver’s License
• Minimum of 1 year experience working with the intellectual disability population.

Preferred 

• Hold Current Clinical Practitioner license by the State of IL
• Master's Degree in Human Services Field
• Advanced Sign Skills
• Knowledge of Deafness and Deaf Culture
• Have an understanding of the impact of hearing impairment on the social and emotional development of deaf and hard of hearing individuals.
• Possess valid Driver’s License
• Minimum of 1 year experience working with the intellectual disability population.

Job Requirements:

• Experience working with individuals with disabilities
• LPN License
• Strong organizational and written documentation skills
• Ability to complete a background check
• CPR/First Aid certification
• Experience with medication management

Job Responsibilities:

• Develop Service Plans for all Adult Clients in accordance with regulatory standards.
• Schedule and conduct all Service Plan Reviews.
• Conduct client assessments annually and as needed.
• Develop and monitor Behavior Modification Plans as needed. Assure that the Service Plan and programming meets the needs of the clients served and document all team meetings.
• Complete monthly reports for Service Plan Implementation.
• Act as a Liaison with all Center programs to ensure continuity of services for Adult clients.
• Provide Crisis Intervention as needed.
• Provide therapy to assigned caseload of clients.
• Ensure that the Client’s Record satisfies the requirements of the funding agency.
• Fulfill the duties of Qualified Intellectual Disability Professional (QIDP) as defined by DHS Rule 115 and Rule 120 services.
• Fulfill the duties of Qualified Mental Health Professional (QMHP) as defined by DHS Rule 132 services.
• Completes other duties as assigned

Residential Care Workers / DSP

We are currently hiring Mental Health Workers and DSPs for our adult residential program located in Northbrook and Glenview. The Center on Deafness serves adults who are Deaf and have an intellectual disability or mental health needs. Full time positions include benefits. Qualified candidates should possess the ability to demonstrate care and compassion, the desire to help individuals reach their full potential, and the willingness the learn sign language.

Job Types: Full-time, Part-time
Pay: From $17.00 per hour

Shifts Available:
Day Shifts: Saturday and Sunday 9:00 - 4:00
Evening shifts: Sunday - Saturday 3:15 - 10:45
Overnight Shifts Monday - Saturday 10:45pm - 8:45am

Job Duties Include:

• Provide an environment where individuals feel safe and supported
• Supervise clients to ensure safety and well-being
• Assist and teach ADL skills such as food preparation, laundry, and other household tasks
• Facilitate community outings; including driving
• Lead individual and group skill-building activities in the areas of daily living, social skills, and community integration.
• Encourage positive relationship building
• Implement treatment plans
• Timely completion of all required documentation
• Ensure the home meets quality standards for cleanliness

Job Requirements:

• Must possess a valid driver's license
• Minimum high school diploma
• Experience is preferred, but will train
• Sensitivity, awareness, and understanding of the client's needs
• Ability to handle disruptive behaviors in a calm, professional manner
• Ability to pass a background check
